63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was conducted today from 10:00am to 12:45pm with the Director. There were 72 children in care, ranging from three to five years of age, supervised by 12 staff. The children were observed in free play in centers, visiting "Santa's Shop" to pick presents for their families, and listening to a story while waiting for their turn to shop. Seven children?s records and four staff records were reviewed. The Center has all staff with current certification in CPR and First Aid, as well as trained in Daily Health Observation. One medication and authorization form were reviewed and were in compliance and the center has four staff current in Medication Administration Training (MAT). The emergency drill log was reviewed and was in compliance. The last documented evacuation drill was dated 11/20/19 and the last documented shelter-in-place drill was dated 6/7/19. The most recent Fire Inspection on file was dated 6/12/19 and the most recent Health Inspection on file was dated 12/11/19. Lunch today was sloppy joes, corn, apple slices, and milk. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of seven children's records, the center did not obtain documentation of a physical examination by a physician within one month after attendance. Evidence: Child A, with a start date of 8/12/19, did not have documentation of a physical in their record.

Plan of Correction: We will request this from the parent.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances, such as cleaning materials, were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In the three-year-old class, there was a container of antiseptic wipes on an open shelf. 2. In a second three-year-old class, there was a spray bottle labeled bleach and water, two sanitizer sprays, two aerosol sanitizer sprays, and one carpet cleaner in an unlocked upper cabinet. 3. In the four-year-old class there was a container of disinfectant wipes on an open shelf. 4. In a third three-year-old class there was a container of antibacterial wipes in an unlocked upper cabinet.

Plan of Correction: We were under the impression that the cleaners only needed to be out of reach of the children. We will make sure they are locked moving forward.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of four staff records and interview with staff, the center did not complete a search of the child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for one staff. Evidence: The sworn statement for Staff A, which was signed on 8/5/19, indicated that the staff had resided in Louisiana within the past five years, but the center had not completed a search from Louisiana.

Plan of Correction: We called today and got the information on how to complete the search and we will send off the paperwork today.
