63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was conducted on 12/12/19 from 9:35 am until 11:45 pm at Bears and Blankets Academy of Early Learning to review the licensing standards. At the time of the inspection 52 children were present with 10 staff. The sample size consisted of five children's records, five staff's records and four diapering ointments. No children were on medication at the time of the inspection. Children and staff were observed during indoor and outdoor free play, art and educational activities, diapering, napping, bathroom break, hand washing, transitions and behavioral guidance. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review and interview, the center failed to obtain documentation of a negative TB screening no later than 21 days after employment and completed within 12 months prior to employment.

Evidence:

1. A review of five staff records was completed. Staff 1's record contained a TB screening dated 9/29/17. Staff 1's start date was 10/14/19.
2. The director verified the dates of the TB screenings. The director believes staff 1 has obtained a new TB screening but she could not locate a copy. Staff 1 is a rehire and the TB on record was from when they worked at the center previously.

Plan of Correction: Staff 1 will be advised to provide a copy of a current TB screening.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure equipment of the center is maintained in a clean safe and operable condition.

Evidence:

Mats used by children during nap are torn at the seams exposing the absorbent sponge.

Plan of Correction: The mats will be repaired or replaced.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the center failed to ensure children's hands were washed with soap and running water after toileting.

Evidence:

In the nursery, staff 1 did not wash an infants hands after diapering.

Plan of Correction: The procedure for diapering will be reviewed with staff 1.

Standard #: 22VAC40-185-520-A
Description: Based on observation, the center failed to ensure over-the-counter skin products were used in accordance with the manufacturer's recommendations.

Evidence:

A tube of diaper ointment was found in the toddler classroom that had expired 10/19.

Plan of Correction: The diaper ointment will be sent home with the parent today. Staff will check all diaper ointment to ensure they are not expired.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review and interview, the center failed to obtain a fingerprint-based criminal history determination letter prior to hire for one out of five staff.

Evidence:

1. The record for staff 1 (start date 10/14/19) has a fingerprint-based determination letter dated 11/15/19.
2. The director stated staff 1 is a rehire and the director thought she has a determination letter from when she worked at the center previously. Once she realized she did not, staff 1 got the determination letter.

Plan of Correction: In the future all staff, including rehires, will be required to obtain fingerprint-based criminal history determination letters prior to hire.
