Comments:
An unannounced monitoring inspection was conducted on Wednesday, December 4, 2019 to determine the center?s compliance with licensing standards. The inspector arrived at 9:35am and concluded the inspection at approximately 1:15pm. The center?s director and assistant director were present and assisted the inspector throughout the inspection. The census for today?s inspection consisted of 151 children in the direct care of 21 staff members. Additional staff were on site and available to the children in care. Upon arrival, the children were engaged in a variety of activities. Each classroom was inspected and formal observations were completed. The center is equipped with age and stage appropriate materials and equipment for the children?s use. All areas where the children receive care, inside and outside, were inspected. The center had the following posted: license, daily schedules, emergency numbers, evacuation maps, menu, and various parent information. Today?s lunch consisted of Salisbury steak with gravy, peas, applesauce, and milk. The center?s first aid kit and non-emergency supplies were inspected and found complete. During the inspection, ten children?s records and ten staff records were reviewed. Medication is administered and five medications were reviewed today. The most recent evacuation drill was documented on 11/25/2019. The most recent shelter-in-place drills were documented on 06/10/2019. One more shelter-in-place drill has been scheduled and must be completed by the end of the year.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation of the center on 12/04/2019, the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence: 1) During the inspection of the infant playground, the licensing inspector observed a plastic shed with a door that had become detached. The detached door was observed on the ground where the children play.

Plan of Correction: Per the Center: The door was reattached to the shed while the inspector was on site. A work order was already in place requesting for the shed to be removed from this playground by January 1st. Moving forward, storage units will no longer be used in the infant playground.

Description: Based on review of 10 staff records and interview on 12/04/2019, the center did not request, within 30 days of hire, a search of out-of-state founded complaints of child abuse for two staff members who have lived outside of Virginia within the last five years.

Evidence: 1) The sworn statement for Staff #1, hired on 8/13/2019, indicated the staff member had lived outside of Virginia within the last five years. The records did not contain documentation of a requested founded complaint of child abuse from the state the staff member indicated she had lived in within the last five years.

2) The sworn statement for Staff #5, hired on 5/07/2019, indicated the staff member had lived outside of Virginia within the last five years. The records did not contain documentation of a requested founded complaint of child abuse from the state the staff member indicated she had lived in within the last five years.

3) During interview, a member of management reported the out-of-state searches of founded complaints of child abuse were not completed for Staff #1 or for Staff #5.

Plan of Correction: Per the Center: HR is working on getting the background checks and verification will be sent to licensing within 10 days. In the future, management will double check sworn disclosure statements to see if an individual has lived out of state. The requests for out-of-state founded complaints will now be managed on site.
