Standard #: 22VAC40-185-70-A Description: Based on record review, in two of two staff records reviewed, the licensee did not ensure that each staff record was documented with all required information.
Evidence: The Program Director confirmed that the following information had not been documented in each staff person's record:
Staff #1-there was not an address for a person to contact in the event of an emergency and one reference check taken over the phone did not have the firm contacted, the results and the signature of the person making the call.

Standard #: 22VAC40-80-120-E-2 Description: Based on observation and staff interview, the licensee did not ensure that the findings of the most recent inspection of the facility.
Evidence: The Program Director confirmed that the findings of the inspection conducted on October 31, 2019 were not posted in the facility.Plan of Correction: Moving forward all licensing inspection documents will be posted.
