Comments:
An unannounced complaint inspection was conducted from 12:00pm-1:45pm to investigate a complaint that came into our office regarding care and protection, children/staff records, ratios, physical plant, medications, and concerns regarding child illness. Based on all available facts reviewed, the complaint regarding ratios was founded and violations unrelated to the complaint were also observed. Violations:
Standard #: 22VAC40-185-350-C
Complaint related: No
Description: Based on observation, staff did not ensure that the staff-to-children ratio applicable to the youngest child in the group was applied to the entire grouping.
Evidence:
1. the Toddlers and Two's classes were combined for nap time in the Two's room. There were 6 toddlers (under 2 years of age) and 10 two year old children, with only Staff #1 supervising from approximately 12:50pm-12:59pm, leaving the classroom out of ratio during the designated rest period.
2. At 12:50pm Staff #1 stated "I am waiting for the other Teacher to come back to the classroom."At 12:59pm, a second staff member returned to the classroom.

Plan of Correction: We have made it clear to our staff the staff to children ratio's importance. They will call the office staff if they need to leave their room for any reason, that all rooms should stay in ratio at all times. More staff has been and will be hired.

Complaint related: No
Description: Based on observation, cots used by children during the designated rest period did not have linens consisting of a top and bottom cover.
Evidence:
1. One cot was observed to be missing a bottom cover. There was a child resting on the cot with a top cover only.

Plan of Correction: Notes has been sent home to all parents and asked to remember sheets as well. Staff will use the school's extra sheets if parents forget to bring their own.
