Comments:
An unannounced monitoring inspection was conducted on 10/22/19 in response to a self-reported incident regarding a child being fed the wrong bottle of formula. Based on record review, staff interviews, and a review of the center?s policies, violations are noted on the Violation Notice. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of children?s records and staff interview, the facility did not ensure that each child has a physical examination by or under the direction of a physician prior to or within 30 days of attendance.
Evidence:
1. Child A (start date: 9/12/19) did not have documentation on file of a physical examination by or under the direction of a physician.
2. Staff #3 confirmed that they have not received the physical examination for Child A from the parent.

Plan of Correction: Dad provided a physical on 11/22/19.

Standard #: 22VAC40-185-40-E
Description: Based on record review and staff interview, the center failed to ensure that the operational responsibilities of the licensee shall include, but not be limited to, ensuring that the center's activities and services are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards.
Evidence:
1. The ?Name to Face- Bottle Feeding? policy for employees provided by the center states that ?Step 3: After the bottle is warmed. You show the bottle to your co-teacher and point to the baby that you are intending to feed. Saying ?I am going to feed (child?s name? this formula bottle labeled ?child?s name?.
2. On 10/17/19 at approximately 4:00pm, Staff #1 removed a bottle from the bottle warmer and began feeding it to Child A).
3. Staff #2 stated that a ?name to face? check was not observed before Staff #1 gave the bottle to Child A.
4. Staff #1 stated ?I grabbed a bottle of formula that already was in the warmer and started feeding Child A?.

Plan of Correction: Retrained Staff #1 on name to face bottle feeding on 10/18/19.

Standard #: 22VAC40-185-340-A
Description: Based on staff interviews conducted, staff did not always ensure care and protection of children.
Evidence:
1. On 10/17/19 Staff #2 stated that ?Staff #1 kept saying that Child A does not want their bottle. I looked and noticed that it was another infant?s bottle. I told Staff #1 that it was another infant?s bottle. There was about 1 oz of formula missing from the bottle when Staff #1 stopped feeding the formula in the bottle to Child A?.

Plan of Correction: Retrained Staff #1 on name to face bottle feeding on 10/18/19.
