63.2(17) License & Registration Procedures

An unannounced Monitoring Inspection was conducted today. At the time of entrance there were 2 staff present with 4 preschool age children. The sample size consisted of 4 children's records and 2 staff record. Children were observed playing freely in the classroom and eating snack. No medications are being administered. The inspection began at approximately 10:20 am and ended at 11:55 pm.

Standard #: 22VAC40-185-130-A
Description: Based on review, facility did not ensure one of four children's records has an immunization record prior to beginning employment.

Evidence: The record for child #2 (start date September 3, 2019) did not have an immunization document.

Plan of Correction: The director will send the immunization documentation to the parent .

Standard #: 22VAC40-185-140-A
Description: Based on record review, in one of four children's records reviewed, the licensee did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.

Evidence: Staff #1 verified that there was no documentation of a physical examination in the record for Child #2 (start date 9/03/19).

Plan of Correction: The director will send the immunization documentation to the parent .

Description: Based on record review and staff interviews, in five of five children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information.

Evidence: Child #4 did not have the following information in the record mother's name of employer and work phone number and a second emergency contact. .

Plan of Correction: Director will get the information today from parent/

Standard #: 22VAC40-185-70-A
Description: Based on records review and staff interviews, the licensee did not ensue there was a complete record with all required information for all staff.

The record for staff #2 did not include documentation that two or more references as to character and reputation as well as competency were checked before employment.

Plan of Correction: The director will complete references today.

Standard #: 22VAC40-185-260-A
Description: Based on documentation review and staff interviews, the licensee did not obtain annual fire inspection report for appropriate fire officials.

Evidence: The most recent fire inspection available for review was dated 11-01-17.

Plan of Correction: The director will contact the Fire Marshall today.

Standard #: 22VAC40-191-60-C-2
Description: Based on one of four records reviewed , the licensee did not ensure staff had a central registry finding within 30 days of employment or volunteer service.

The record for staff #2 (date of hire 08/19/2019) did not include documentation of central registry results with in 30 days of employment.

Plan of Correction: The staff #2 will have it filled out by tomorrow and will send it .

Standard #: 63.2(17)-1720.1-B-2
Description: Based on one of two records reviewed and interview, licensee did not ensure an Office of Background Investigation fingerprinting determination letter was received prior to staff beginning employment.


Staff #2 (date of hire 8/19/2019) did not have a fingerprinting determination letter of results until 11/20/2019.

Plan of Correction: The director will not hire any staff until the results fingerprinting is received.
