Comments:
An unannounced monitoring inspection was conducted today from 12:45pm through 4:00pm. There were 83 children in care ages three through four years old with 12 teachers. Children records and staff emergency contact information were reviewed. Required postings, annual inspections, emergency evacuation drills and first aid kit were reviewed. Children were observed in teacher and self-directed activities including restroom routines, transitions hand washing, snack, nap and play activities. There were not any violations identified during this inspection. Violations regarding staff records and cot spacing were identified and appear on the violation notice. The violations were discussed with the program director at the conclusion of the inspection.

Description: Based on record review and interview the center failed to demonstrate that each staff record contains all required documentation.
Evidence:
1 - On site documentation did not include emergency contact information for staff 2.
2 - Staff 3 confirmed that there was not documentation of emergency contact information for staff 2.

Standard #: 22VAC40-185-440-E
Description: Based on observation the center failed to demonstrate 12 inches between occupied cots.
Evidence:
1 - In classroom "F" there were four children napping on cots, next to each other, however, the cots have less than 12 inches between them.
2 - Staff 1 confirmed that four children were napping with less than 12 inches of space between the occupied cots.
