Comments:
An unannounced monitoring inspection was conducted on 12/17/19 from 11:00am until 12:30pm. At the time of entrance, there were 24 children in care with 6 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed eating lunch, watching a Christmas movie and resting quietly during nap time. Restroom and hand washing procedures were also reviewed. First aid and emergency supplies, documentation of emergency practice drills, children's injury reports and center postings were reviewed. The center does not administer medication or provide transportation to children in care. Three violations are identified on the violation notice in the areas of administration, physical plant and background checks and were discussed with the assistant director during the exit interview.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review and interview, the center did not ensure that at least every two years from the date of the initial screening or test staff members shall obtain and submit the results of a follow-up tuberculosis screening.

Evidence:
1. The most recent tuberculosis screening on record for staff 2 is dated 8/31/17.
2. Staff 2 confirmed that her tuberculosis screening is expired.

Plan of Correction: The center responded with the following: Staff 2 will obtain a new tuberculosis screening by the end of the week.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that hazardous substances, such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the school-age bathroom, there were two containers of bleach in an unlocked wicker cabinet.
2. In the bathroom by the lunch area, there was a container of disinfectant spray and a container of Comet with Bleach Powder in an unlocked cabinet under the sink.

Plan of Correction: The center responded with the following: The cleaning materials were placed in a locked location during the inspection. Staff were reminded that all hazardous materials need to be kept in a locked place at all times.

Description: Based on record review and interview, the center did not ensure that staff who have lived outside of the state of Virginia within the past 5 years obtain a central registry check from that state.

Evidence:
1. Staff 1 (date of hire: 3/14/18) indicted on her sworn statement or affirmation that she has resided in the state of California within the past 5 years. The record for staff 1 does not contain the results of an out-of-state central registry search for the state of California.
2. The center director confirmed that the record for staff 1 does not contain the results of an out-of-state central registry search for the state of California.

Plan of Correction: The center responded with the following: The wrong form was filled out and sent to the state of California. The correct form will be filled out and sent to California by tomorrow. Once received, the results will be placed in staff 1's record.
