Comments:
A complaint investigation was conducted on November 13, 2019 regarding insufficient meals requirements, outdoor play, and forbidden actions. Interviews were conducted, and documentation was reviewed. There was not a preponderance of evidence to support the allegations regarding outdoor play and forbidden actions; however, the complaint was deemed valid with regard to insufficient meal components being served. The complaint related violation and two non-related violations are identified in the Violation Notice.

Description: Based on observation, the center did not maintain one staff member for every five children, for children 16 months old to two years.

Evidence:
1. From approximately 11:10am to 12:45pm, the Toddler classroom (children ages 18 months-3 years) had 11 children and 2 staff members.
2. At approximately 10:55am, upon Staff B returning to the classroom, the Toddler classroom had 11 children and 1 staff member.

Description: Based on observation, the center did not ensure that children's hands were washed with soap and running water or disposable wipes before and after eating meals or snacks, and the staff did not wash their own hands before feeding or helping children with feeding.

Evidence:

1. In the Toddler classroom (children ages 18 months-3 years), the staff did not have the children wash their hands upon completing circle time and beginning to eat lunch.
2. In the Toddler classroom, Staff A and Staff B did not wash their hands before assisting children with feeding.

Description: Based on observation, the center did not follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).

Evidence: In the Toddler classroom (children ages 18 months-3 years), 1 of 3 children served school-provided lunch was not offered milk with their lunch, as required by the Child and Adult Care Food Program of the USDA.
