Comments:
An unannounced renewal inspection was conducted today from 11:00am-12:50pm. There were 24 children (2-5yrs.) supervised by 6 direct care staff. The physical plant, 3 staff records, 3 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing in home living, with bristle blocks, tracing, playing with puzzles, and learning letters "F" and "G". Toileting and hand-washing procedures were also observed. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-60-A
Description: Based on record review, 1 of 3 children's records did not contain documentation of all required information.

Evidence: Child #3's (age: 2 years old) record did not contain documentation either parent's work information and previous schools attended.

Plan of Correction: We will do a follow-up with new students to review their paperwork after they start, and will use a checklist to make sure all items are completed.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence: An unlocked storage closet in the hallway contained a lighter and a bottle of hydrogen peroxide.

Plan of Correction: Corrected during inspection. All staff will be reminded to check doors to make sure they are locked.

Standard #: 22VAC40-185-550-M
Description: Based on record review, 2 of 5 injury reports did not include documentation of all required information.

Evidence:
1. Two injury reports did not contain documentation of the date of parent notification.
2. One injury report did not contain documentation of the time of parent notification, and one did not contain documentation of the method of parent notification.

Plan of Correction: We will have a new form.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, 1 of 3 staff records, did not contain documentation of central registry findings within 30 days of employment.

Evidence: Staff C's (date of hire: 10/28/2019) record did not contain documentation of a central registry finding.
The employee file did contain documentation of a central registry request that was mailed on 10/28/2019.

Plan of Correction: We will follow-up with OBI.
