Consultation provided on standard 40F: Every center shall ensure that any advertising is not misleading or deceptive as required by 63.2-1713 of the Code of Virginia.

Consultation provided on standard 60A6: Appropriate legal paperwork shall be on file when the custodial parent requests the center not to release the child to the other parent.

Comments:
An unannounced monitoring inspection was conducted today with a teacher and later the director. Three children were present with two staff at the beginning of the inspection. The children were engaged in free play and then separated into their classrooms where some were eating breakfast, which consisted of milk, cereal and mandarin oranges. Six staff and children's files were reviewed today. Eight medications for four children were reviewed as well.

Description: Based on a review of six staff files and an interview with the director, it was determined that one staff did not have an up to date tuberculosis test every two years.
Evidence:
Staff A's tuberculosis test is dated 3/30/17 and ha e not been updated.

Plan of Correction: The staff will get the tuberculosis statement.

Standard #: 22VAC40-185-510-C
Description: Based on a review of medication and an interview with the director, it was determined that a medication did not have up to date parental authorization.
Evidence:
Child A has a cough, mucus and congestion medication at the center and the parental authorization for this medication was dated 11/11/19 and have not been renewed every 10 work days.

Plan of Correction: I will get the medication authorization.

Standard #: 22VAC40-185-550-D
Description: Based on a review of the escape drill log, it was determined that there was no monthly escape drill for November 2019.

Plan of Correction: I will remember to do monthly escape drills.
