63.2(17) License & Registration Procedures

Comments:
An unannounced renewal inspection was conducted with the center director on December 13, 2019 from 1:00 p.m. to 2:15 p.m. to determine compliance with the licensing standards. There was one child present, age 23 months, who was asleep at rest time. Two applicants' files and one child's file were reviewed - please see Supplemental Summary for details. The last fire inspection was conducted 9/24/2019. The last health inspection was conducted 10/09/2019. Evacuation drills have been conducted monthly; the last shelter in place drill was conducted 10/2/2019. There are no children receiving medication at the facility. Standard #: 63.2(17)-1721.1-B-2
Description: Based on review of two administrative files, the facility failed to have documentation that each applicant/agent submitted to fingerprinting. Evidence: Applicant 1 and Applicant 2 did not have documentation of fingerprint results.

Plan of Correction: Applicant 1 and Applicant 2 will schedule an appointment with Field Print as quickly as possible.

Description: Based on review of two administrative files, the facility failed to have documentation of a child abuse and neglect registry check or equivalent maintained by any other state in which the individual has resided in the preceding five years. Evidence: Applicant 1 and Applicant 2 stated they had lived in another state in the past five years, but did not have documentation of a child protective services check from that state as required.

Plan of Correction: Applicant 1 and Applicant 2 will submit a CPS check to the other state lived in as soon as possible.
