An unannounced monitoring inspection was conducted today.
Eighteen children were present in the primary class with two staff.
The children were working independently.
Five children's files, and five staff files were reviewed for compliance.
You reported that the pre-primary class is not currently operating.

Standard #: 22VAC40-185-330-B Description: Based on a tour of the playground, the facility failed to ensure that there is six inches of resilient surface material under climbing equipment.
Evidence:
1. On the playground, there is less than two inches of mulch under the slides.Plan of Correction: The program director will ensure that the mulch is raked. More mulch will be purchased if needed.
Standard #: 63.2(17)-1720.1-A Description: Based on the review of five staff files, the facility failed to conduct a repeat background check on an employee.
Evidence:
1. The central registry clearance on file for staff #1 is dated 10/31/14.Plan of Correction: The program director will ensure that the repeat central registry clearance is conducted.
