Comments:
A Subsidy Health and Safety inspection was conducted on December 18, 2019 to review supplemental health and safety requirements. Nine staff were present with thirty-five children during the tour of the facility. Additional staff were on site for administrative and kitchen duties. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-510-B
Description: Based on review and observation, vendor did not ensure a written hard copy record of daily attendance for each grouping of children that documents the arrival and departure of each child in care as it occurs.

Evidence: The following was observed and reviewed during the tour of the facility:
1. In the 3's & 4's room, ten children were observed present and twelve children were noted signed in on the attendance sheet for the room. Staff stated two children had left for school and they have not been signed out.
2. In the toddler 2 room, eleven children were observed present and eight children were noted signed in on the attendance sheet for the room.

Plan of Correction: Director will talk to staff to have them reiterate to parents about signing their children in each day. Director will have staff sign children out once they leave the building for school.

Standard #: 22VAC40-665-580-B
Description: Based on review and interview, vendor did not ensure 1 of 5 staff who work directly with children complete Virginia Preservice Training for Child Care Staff within the first 90 days of employment.

Evidence: The record for staff #3 (start date September 16, 2019) did not have documentation of completed Virginia Preservice Training. Director confirmed staff member has not completed the training.

Plan of Correction: Director will have staff complete the training.

Standard #: 22VAC40-665-650-E
Description: Based on interview, vendor did not ensure required policies and procedures are developed and written.

Evidence: Director stated she does not have a written policy and procedure that describes how the vendor will ensure that each group of children receive care by consistent staff or team of staff members.

Plan of Correction: Director will write the policy.
