Comments:
An unannounced monitoring inspection was conducted on December 18, 2019 from approximately 9:00 am to 12:00 pm. Nine staff were present with thirty-five children. Additional staff were on site for administrative and kitchen duties. Five children and five staff records were reviewed.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observations, the center did not ensure all areas and equipment were maintained in a clean, safe, and operable condition.

Evidence: The following observations were completed during the inspection:
1. In Infant classroom, there was a green cushion torn on one side with foam padding exposed accessible to the children in care.
2. In Toddler 1 classroom, two stuffed animals were torn with the foam padding exposed accessible to the children in care.

Plan of Correction: The items will be removed from the classrooms.

Standard #: 22VAC40-185-280-B
Description: Based on observations, the center did not ensure hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In Toddler 1 classroom, there was an unlocked closet with two cleaners with the labels "caution" and "keep out of reach of children" observed in a plastic blue bin. A bottle of hand sanitizer with the labels "warning" and "keep out of reach of children" on the window ledge.
2. In Toddler 2 classroom, a bottle of hand sanitizer on the shelf with labels "warning" and "keep out of reach of children".

Plan of Correction: All chemicals will be removed and a lock box will be purchased for classrooms.

Standard #: 22VAC40-185-500-A
Description: Based on observation, facility did not ensure staff and children wash their hands after a diaper change.

Evidence: Staff was observed changing a child's diaper in the toddler 1 classroom. The staff and the child's hands were not washed after the diaper change.

Plan of Correction: Director will retrain staff on hand washing procedures.

Standard #: 22VAC40-185-550-M
Description: Based on review, facility did not ensure 1 of 3 injury reports has documentation of required information.

Evidence: An injury report completed on August 19, 2019 did not document staff present.

Plan of Correction: Director will remind staff to fill out forms completely.
