Comments:
An unannounced monitoring inspection was conducted on 12/2/19 from 11:05 am until 2:45 pm at Charlottesville Waldorf School to review the licensing standards. At the time of the inspection no children were in care. The sample size consisted of five children's records and five staff's records. Violations from the previous inspection were reviewed for current compliance. Three repeat violations were found. Violations were found during this inspection and are documented on the violation notice.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and interview, the center failed to maintain a complete record for each child enrolled.

Evidence:

1. The records for five children's records were reviewed. None of the records contained an address for either emergency contact. The work phone number for both parents was missing for child 1, child 2 and child 3. The records for child 4 and child 5 only contained the work phone number for one parent.
2. The administrator verified they do not have the required information.

Plan of Correction: The office manager has contacted the families and is updating the applications to receive the proper information.

Standard #: 22VAC40-185-240-C
Description: Based on record review and interview, the center failed to ensure staff annually attended at least 16 hours of staff development activities related to child safety and development and the function of the center.

Evidence:

1. The records of five staff were reviewed. Staff 1 was to complete 16 hours of annual training between the time frame of 8/15/18 and 8/14/19. The record for staff 1 contained documentation of completing one hour and thirty minutes of training. Staff 4 was to complete 16 hours of annual training between the time frame of 9/18 through 9/19. The record did not contain documentation of training being completed.
2. The administrator was unable to provide documentation staff 1 and staff 4 completed additional training.

Plan of Correction: The center is discussing training opportunities for staff to be completed by February.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure hazardous substances such as cleaning materials are kept in a locked place using a safe locking method that prevents access by children.

Evidence:

In the EC Aftercare room a spray bottle of bleach/water solution and disinfectant was found on an open shelf in the bathroom used by children.

Plan of Correction: The administrator will talk to staff about locking substances during the staff meeting.

Description: Based on observation and interview, the center failed to ensure the nonmedical emergency supplies worked properly.

Evidence:

1. The battery operated radio in the Grades Aftercare room did not have working batteries.
2. The administrator verified they did not have working batteries for the radio.

Plan of Correction: Batteries have been replaced.

Description: Based on observation and interview, the center failed to post the most recently issued license.

Evidence:

1. A current license was not posted at the center.
2. The administrator verified they do not have a current license posted.

Plan of Correction: The license and violations will be posted once the 12/2/19 visit is finalized.

Description: Based on observation and interview, the center failed to post the most recent inspection of the facility.

Evidence:

1. The inspection posted in the EC Aftercare room was dated 4/16/18. The most recent inspection was dated 5/23/19.
2. The administrator verified the most current inspection was not posted.

Plan of Correction: The license and violations will be posted once the 12/2/19 visit is finalized.
