An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted from 7:30 a.m .to 8:40 a.m. in conjunction with a licensing inspection. Three staff records were reviewed and all staff have current CPR, First Aid and Virginia Pre-Service Training. Two allergy action plans were reviewed. Lockdown drills were posted and up to date. SHSI requirements were reviewed.

Standard #: 22VAC40-665-520-B-11 Description: Based on review of documentation, it was determined that the vendor did not obtain documentation of an allergy care plan for each child diagnosed food allergy from the child's physician. Evidence: Child #6 has a diagnosed food allergy for peanuts and nuts and there was no documentation of an allergy action plan from the child's physician. Plan of Correction: This information will be requested from the parents again.
