Comments:
An unannounced monitoring inspection was conducted beginning at 9:15 a.m. and ending at 1:15 p.m. with the director. There were 102 children in care ranging in age from 5 months to 5 years old under the care and supervision of 18 staff. Infants were observed during diapering, play time, bottle feeding, story time and rest. Toddlers were observed during free play. Two year olds were observed during centers that included flashcards and blocks. Preschoolers were observed during centers that inlcuded a sensory table, home living and playdoh. Preschool B was observed during morning circle time. PreK (four year olds) were observed during centers that included journals, an art activity about the letter C, Transitional Kindergarten (five year olds) were observed during an interactive lesson with the smart board.

The outdoor play areas and playground were observed. 10 children's records, 6 staff records and a sample of 7 medications and authorizations were reviewed. Fire drills were reviewed and up to date.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas and equipment of the center were maintained in safe, operable condition. Evidence: In the bathroom in the Two year old classroom, there was a potty seat adapter that had tears in the nonabsorbent material exposing absorbent material inside making it unable to be cleaned and sanitized. In the Preschool B bathroom, the toilet on the right of the bathroom is marked out of order with tape and plastic.

Plan of Correction: A new toilet has been ordered for both bathrooms prior to the inspection. The potty seat was discarded during the inspection.

Standard #: 22VAC40-185-510-C
Description: Based on review of documentation and interview with staff, it was determined that center policy regarding prescription medication was not followed. Evidence: In the Two year old class, there was an unlabeled prescription hydrocortisone in a ziploc bag and there was no authorization from the parent or prescription label.

Plan of Correction: The parents will be reminded that all ointments and prescription medication must be checked in at the main office and have parental authorization completed before being left at the center. Ointment was removed from the classroom and the necessary documentation will be obtained.

Standard #: 22VAC40-185-520-A
Description: Based on observation and review of over the counter skin products, it was determined that requirements including expiration date of the product were not met. Evidence: In the Two year old class, there was an ointment for Child #2 that expired 9/2019.

Plan of Correction: The ointment was discarded during the inspection.

Standard #: 22VAC40-185-520-C
Description: Based on observation, it was determined that requirements for diaper ointments were not followed. Evidence: In the Two year old class, there were four ointments that were not labeled with the child's name. There was an A&D ointment that did not have an authorization signed by the parent.

Plan of Correction: All parents will be reminded that diaper ointments must be checked in at the main office. All ointments that were not labeled have been labeled or removed.

Standard #: 22VAC40-185-560-N
Description: Based on observation, children were permitted to drink while walking around. Evidence: In the toddler classroom, a child was observed walking around drinking a sippy cup of water.

Plan of Correction: Staff were reminded to have children sit while drinking.
