An announced non-mandated record review inspection for the Albert Harris Elementary Boys and Girls Club After School Program was completed on 12/19/2019. The inspector reviewed 4 staff records during the inspection today. The inspector arrived at 12:00 p.m. and departed at 2:00 p.m. Thank you for your time today. Standard #: 22VAC40-191-60-C-2 Description: Based on record review and discussion with the staff, the facility failed to obtain the central registry finding within 30 days of employment.
Evidence:
1. The record for Staff #1 had a documented hire date of 10/08/2019; the date on the central registry findings was 11/12/2019.
2. The record for Staff #3 had a documented hire date of 10/08/2019; the date on the central registry findings was 11/12/2019.Plan of Correction: Will document if findings are not returned within 30 days.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review and discussion with staff, the facility failed to obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him.
Evidence: The record for Staff #3 contained documentation of having resided outside of the state of Virginia within the preceding 5 years; there was no documentation of a request to that states central registry or equivalent available for review. .Plan of Correction: A request will be made.
