Technical Assistance:
We discussed the requirements for hand washing and children's records. The risk assessments for violations were provided.

We reviewed the requirements for authorizations for skin products, over-the-counter medication, resilient surfacing, injury records and the storage of soiled linens.

Comments:
Thank you for your assistance during today's unannounced monitoring inspection conducted from 10:45 AM to 3:15 PM. Today, there were seventy-two children, infancy through five years, with eleven staff. I viewed daily routines, staffing, supervision, interactions with children, equipment, infant care, posted information, injury records, menu, nutrition, emergency supplies and drill records, medication, agency inspections, insurance, emergency planning, seven records for children, seven staff records, staff qualifications. staff training, indoor and outdoor areas. The children have enjoyed planned activities, stories and discussions about various holidays of the season. The infants were provided opportunities for activities and movement on the floor, and interactions with caregivers. Regular outside experiences have been provided, weather permitting.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of records for children, and an interview with administrative staff, the center failed to maintain records which contained all applicable required information. Evidence: The registration form used by parents only has one section for completing information about one parent, including place of employment and work phone number. At times, the second parent information was added to the emergency contact list on the form or was in file on previously used files. The record for child 1 and 2 did not have the place of employment for the second parent. The work phone number was missing for the second parent of child 1.

Plan of Correction: The director immediately began working on a form used for updating information as a supplemental record completed by parents. This form has space for information about two parents, including employment information. The form will be emailed to parents and obtained for children's records. It will be used as children are enrolled.

Standard #: 22VAC40-185-500-A
Description: Based on observation of diaper changes, and an interview with staff, staff did not ensure that they always wash their hands with soap and running water after a diaper change. Evidence: During diaper changes before lunch in the toddler room, staff member 8 was seen completing a diaper change and then handling keys, the sink cabinet handle, the bathroom door knob and step stool before washing hands at the sink.

Plan of Correction: Staff will make sure all items needed after a diaper change are obtained in advance. The director will review the diapering procedures and hand washing requirements with staff member 8. Staff will always wash hands after each diaper change and before handling items in the room. Administrative staff will conduct periodic observations of diaper changes to ensure that staff follow the required procedures.
