Technical Assistance:
Consultation today included presence of a jar of peanut butter even when no diagnosed allergies are in that particular classroom.
We also discussed age appropriate decorations in infant and toddler rooms, Sworn Statements, cribsheets and training for new staff. We also discussed the snack standards requirement regarding the USDA standards for the menu.

Comments:
An unannounced renewal inspection was conducted today from 9:45 am - 1:15 pm. There were 32 children present with 9 staff. I observed children coloring worksheets, putting together puzzles, listening to seasonal music and naptime. Outdoor areas were not inspected due to weather. Violations:
Standard #: 22VAC40-185-40-J
Description: Based on a review of records and staff confirmation today, the center's injury prevention plan was not updated at least annually based on a review of injury reports as required.

Plan of Correction: I will review the injury reports and make changes that are based on the reports. We do that now but it is not documented.

Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records today, not all documentation for lead staff was available.
Evidence:
Staff # 1 did not have documentation of her education available for review.

Plan of Correction: She will bring it in.

Standard #: 22VAC40-185-270-A
Description: Based on observation today, there were some areas and equipment in the center observed to not be maintained in clean, safe and/or operable condition.
Evidence:
1. Peeling and cracking paint was observed on walls, windowsills, doorways and cabinets, and cracking wood was observed on the cabinets in classrooms and the diaper table in one classroom.
2. Items not in use were not securely stored in children's areas. A folded metal adult chair and a folded stepladder were leaning against a wall in the toddler room; and feeding tables were stored on an open shelf above the room.
3. One small refrigerator containing children's lunches and sippy cups was very crowded with items, and frost was building on the freezer shelf. A second small refrigerator with baby bottles had a significant amount of frost.
4. A children's bouncy seat had six empty circles from where toys had been attached but were now missing. Another swing seat had material that was stained and badly pilling.

Plan of Correction: We will be getting the rooms painted by the end of January/beginning of February.
We will find better storage areas for large items.
The refrigerators will be defrosted and rearranged.
The bouncy seat will be removed and the cover of the swing will be washed.

Standard #: 22VAC40-185-280-B
Description: Based on observation during today's inspection, hazardous substances were not always kept in locked places as required.
Evidence:
A janitor's closet next to the children's classroom in the hallway was unlocked and floor cleaner, window cleaners, weed killer and other items stored without being locked.
A smaller unlocked closet in the same hallway had a box of baking soda accessible on the floor.

Plan of Correction: We don't have a key to the janitor's closet and the smaller closet - they are property of the church and always kept locked. I was able to lock it from the inside today. However, we will check the lock every Monday to make sure anything inside is not accessible to the children and discuss this with the church..

Standard #: 22VAC40-185-430-B
Description: Based on observation during today's inspection, children were exposed to art materials that were not age appropriate.
Evidence:
In one classroom, toddlers were observed "making ornaments" with a staff member. The ornament was a clear plastic globe with a small metal top, of chokeable size. The staff member was observed assisting toddlers in placing tiny sparkly confetti stars into the globe.
In another classroom, crafts with "googly" eyes were observed hanging above a crib.

Plan of Correction: We will talk to the staff about plans for parent gifts.

Standard #: 22VAC40-185-500-B
Description: Based on observation during today's inspection, some diapering standards were not being met as required.
Evidence:
1. Diapers were observed not being dispose of in a foot-operated manner that ensured exterior surfaces were not touched. One diaper disposal did not have a lid; another was too far to reach the foot pedal.
2. Diapering surfaces were not only used for diapering when parts of children's art projects were placed temporarily on the diapering surface.
3. Diapering surfaces were observed today to not be able to be sanitized properly in one room. One changing diaper table had cracked pieces held together by tape, which did not ensure proper sanitization. The diaper pad was covered with contact paper in a way that left folds and taped areas that were not able to be adequately sanitized.

Plan of Correction: A new changing table will be purchased, with a new diaper pad.
