Comments:
An unannounced monitoring inspection was conducted today from 7:00am to 8:00am with the Director. At the beginning of the inspection there were five children, ranging from five to ten years of age, supervised by two staff. More children arrived throughout the morning and another staff arrived at 7:41am. The children were observed in free play. Five children?s records and two staff records were reviewed. The Center has one staff with current certification in CPR and First Aid, and one staff trained in Daily Health Observation. Four medications and authorization forms were reviewed and the center has one staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed and were in compliance. The last documented evacuation drill was documented 11/20/19 and the last shelter-in-place drill was documented 10/3/19. The most recent Fire Inspection on file was dated 1/25/19 and the most recent Health Inspection on file was dated 8/27/19. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review of five children's records, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child attended the center. Evidence: Child D, with a start date of 10/8/19, did not have documentation of immunizations in their record.

Plan of Correction: There was a copy on site at Hugh Mercer. We received a copy of the immunizations for our records Monday, December 9th, 2019.

Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, the center did not obtain documentation of a physical examination by a physician within one month after attendance for one child. Evidence: Child D, with a start date of 10/8/19, did not have documentation of a physical in their record.

Plan of Correction: There was a copy on site at Hugh Mercer. We received a copy of the immunizations for our records Monday, December 9th, 2019.

Standard #: 22VAC40-185-90--A
Description: Based on documentation review and interview with the director, there was no written agreement between the parent and the center in each child's record by the first day of attendance authorizing emergency medical care should an emergency occur when the parent cannot be located. Evidence: The enrollment forms for Child A, Child B, Child C, and Child D did not have a place for parents to give written authorization for emergency medical care.

Plan of Correction: We now have written authorization for all of the children enrolled.

Standard #: 22VAC40-185-240-D-1
Description: Based on documentation review and staff interview, there was not always a staff member on site with certification in Medication Administration Training (MAT) though the center has agreed to administer medication to children. Evidence: Four emergency medications for three children were observed at the facility, however, there were no staff members on site with MAT certification from 7:00am until 7:41am.

Plan of Correction: There were no children present with allergies the morning of December 6th.
