Comments:
An unannounced monitoring inspection was conducted today from 11:15am to 1:00pm with the Director. There were 24 children in care, ranging from three to five years of age, supervised by four staff. The children were observed having free play in Centers and discussing their Nativity performance this afternoon. Four children?s records and two staff records were reviewed. The Center has staff with current certification in CPR and First Aid, as well as Daily Health Observation. Four medications and authorization forms were reviewed and were in compliance and the center has four staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 7/26/19 and the most recent Health Inspection on file was dated 3/4/19. Standard #: 22VAC40-185-160-C
Description: Based on review of documentation, the center did not ensure that staff obtain and submit the results of a follow-up Tuberculosis (TB) test or screening at least every two years from the date of the first initial screening or test. Evidence: The most recent documentation of a TB test for Staff A was dated 8/21/17.

Plan of Correction: I will have her get that done as soon as possible.

Standard #: 22VAC40-185-80-A
Description: Based on review of documentation, the center did not keep a written record of children in attendance each day. Evidence: In the four-year-old classroom there was no attendance documented for 12/18/19, 12/16/19, and 12/13/19.

Plan of Correction: We had some staff out during that time and other staff covering the class. We will make sure we discuss with the Teachers the importance of the attendance.

Standard #: 22VAC40-185-550-D
Description: Based on interview with the director, two shelter-in-place drills were not practiced per year. Evidence: The Director stated that they have not completed the drills this year.

Plan of Correction: We will make sure we complete two per year moving forward.
