Comments:
The licensing inspector conducted an unannounced monitoring inspection, at which lunch time, play groups, personal hygiene, story time, and nap time were observed. Children were observed; and staff and children files and other documentation were reviewed. Twenty-seven (27) children, ages 6 months-4 years, were found to be in care with 5 staff, and administrative staff. Previous violations were reviewed, and one (1) of the violations from the previous inspection had not been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 11:40 am and concluded at 2:10 pm.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of staff records the facility failed to maintain compliance with this standard as required relating to TB tests or screenings.

Evidence:
Staff record #1, hire date 6/11/19, did not have a TB test or screening as required.

Plan of Correction: The staff will obtain a new TB screening or test immediately and proof will be placed in the file for future review and compliance.

Standard #: 22VAC40-185-280-B
Description: Based on observations the facility failed to maintain compliance with this standard as required relating to locked storage of hazardous items. Non-compliance with this standard was previously cited on 06/10/19 therefore making this a repeat violation.

Evidence:
1.In the hallway leading to the schooler area there was a storage closet unlocked that contained hazardous maintenance items.
2. Directly across the hall, in the same area, there was a staff storage/office area that was unlocked and had numerous cans of paint.
3. The kitchen door, in the same area, was unlocked and contained numerous cleaning products on top of the refrigerator.

Plan of Correction: These areas will be kept locked when not immediately in use, to ensure the safety of the children and future compliance.

Standard #: 22VAC40-185-440-B
Description: Based on observation and discussions with staff the facility failed to maintain compliance with this standard as required related to labeling of cribs.

Evidence:
The LI observed no labels of individual identification on the cribs that the children were actively using during the inspection.

Plan of Correction: The labels were removed for cleaning and maintenance of the cribs and had not yet been replaced. The names will be placed on the cribs immediately for compliance.

Standard #: 22VAC40-185-450-A
Description: Based on observations the facility failed to maintain compliance with this standard as required relating to cot sheets.

Evidence:
The LI observed nap time and there was one cot in the toddler-2 year class without a sheet, and 7 cots in the preschool class without sheets.

Plan of Correction: Cot sheets were immediately obtained and placed on the cots. Staff will be reminded that if the cots are not in full compliance that children may not be on the cots until compliance is met.
