Comments:
An unannounced monitoring inspection was conducted with the director beginning at 10:00 AM and ending at 11:45 AM. There were four, three year old, children in the care of one staff members during the inspection. The children were observed being read a story by the staff member. Five children's records were reviewed in compliance. Five staff records were reviewed and in compliance. There were no medications on site to be reviewed. The emergency evacuation drill log was reviewed. The most recent evacuation drill was documented as 11/20/19. The most recent shelter-in-place drill was documented as 7/5/19. The outside play area was not inspected due to weather conditions. The most recent health inspection was dated 11/14/19. The most recent fire inspection was dated 3/26/19.

Standard #: 22VAC40-185-130-A
Description: Based on a review of documentation and staff interview, the center did not have documentation of a immunization for all children with prior to first day of attendance. Evidence: Child A and child B, start date 7/3/19, did not have documentation of immunizations in their records.

Plan of Correction: The center will request the missing documentation from the parents and placed in the children's records upon receipt.

Standard #: 22VAC40-185-140-A
Description: Based on a review of documentation and staff interview, the center did not have documentation of a physical examination for all children with in one month after attendance. Evidence: Child A and child B, start date 7/3/19, did not have documentation of physical examination in the record.

Plan of Correction: The center will request the missing documentation from the parents and placed in the children's records upon receipt.

Standard #: 22VAC40-185-60-A
Description: Based on a review of documentation, not all of the children's records had the required information present during the inspection. Evidence: Child A and child B's records were missing the second emergency contact.

Plan of Correction: The center will request the missing information from the parents.

Standard #: 22VAC40-185-270-A
Description: Based on observation, not all of the areas inside the center were maintained n a clean condition. Evidence: There was excessive amount of dust on the vent covers in the classrooms. The bathroom in the school aged classroom had a strong urine odor present during the inspection.

Plan of Correction: The bathroom in the school age classroom will be cleaned today. The center will talk to the boys about cleaning up after themselves in the restroom. The vents will be cleaned as soon as possible.
