Comments:
An unannounced monitoring inspection was conducted today from 3:10pm - 5:00pm. There were 39 children in care ages six months through four years old with nine teachers. Children and staff records were reviewed. Required postings, annual inspections, emergency evacuation drills, first aid kit and medication were reviewed. A walking of the center inside and outside was completed. Children were observed engaged in teacher and self-directed activities including restroom routines, transitions hand washing, snack, and inside and outside play activities. Violations regarding supervision and ratio were identified and appear on the violation notice. The violations were discussed with the owner at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-350-A
Description: Based on observation the center failed to ensure that staff are counted in the required staff-to-children ratios only when they are directly supervising children.
Evidence:
1 - Upon arrival, in the cafeteria there were 13 children present. However, staff were moving in and out of the area, as a result the children were left alone and unattended for approximately one minute.
a. Staff 1 was in the kitchen and stated that she was putting away groceries.
b. Staff 2 was also in the kitchen and stated that the other staff had been gone for not more than five minutes.
c. Staff 3 walked out of the cafeteria area and into another classroom.

Plan of Correction: Staff will be reminded and retrained regarding the importance of staying with the group of children

Standard #: 22VAC40-185-350-E-4
Description: Based on observation and interview the center failed to ensure that for children ages three years through school eligibility there is one staff to every ten children.
Evidence:
1 - The preschool classroom there were 14 children ages three and four years old. However, there was only one staff present in the classroom where two staff are required.
a. Staff 2 stated that the second staff assigned to the classroom had stepped out of the room.
2 - In the cafeteria there were 13 children ages three to four years old. However, during snack prep there was one staff consistently present were two are required. Additional staff moved in and out of the space at random.

Plan of Correction: Staff will be retrained and reminded that they must maintain ratios at all times.
