Comments:
A complaint investigation was conducted on December 18, 2019 regarding noncompliance with ratios and bathroom supplies. Interviews and observations were conducted, and documentation was reviewed. There was not a preponderance of evidence to support the allegation regarding bathroom supplies; however, the complaint was deemed valid with regard to noncompliance with ratios. The complaint related violation and one non-related violation are identified in the Violation Notice.

Complaint related: No
Description: Based on observation, the center did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the K-Prep classroom (children ages 3-5 years old), a can of Lysol aerosol spray and hand sanitizer were sitting on top of a teacher cabinet.

Plan of Correction: Lysol spray was locked up in teacher cabinet; Teachers were reminded that all substances must be locked up when not in use.

Complaint related: Yes
Description: Based on record review and staff interview, 18 of 22 days, the center did not maintain one staff member for every 10 children, for children from three years to the age of eligibility to attend public school.

Evidence:
1. Based on a review of the name-to-face records for November 18, 2019 through December 18, 2019, 18 days documented that the classroom was over the required ratio in the K-Prep classroom (children ages 3-5 years old).
2. During staff interview, Staff A stated, "Sometimes if a staff member calls out, the class is out of ratio".

Plan of Correction: We ensure floaters are placed in classroom to maintain ratio.
