*Please ensure that you are using a bleach water solution to clean tables or an approved spray solution and not Clorox wipes to sanitize and clean tables before and after use/meals.
*Please ensure that documentation of each child's first day of attendance is on file.
*When conducting Shelter-in-Place or Lockdown drills, please ensure that you are also completing the required monthly evaucation drill during the same month. Shelter-in-Place and Lockdown drills do not replace the evacuation drills.

Comments:
An unannounced Monitoring Inspection was conducted today from 11:00am to 1:30pm. There were 37 4-5 year old students directly supervised by 4 staff. The physical plant, 4 staff records, 5 children?s records, 2 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, preparing for and eating lunch, and preparing for and taking naps. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-160-A
Description: Based on review, the facility did not ensure that documentation of a negative tuberculosis screening shall be submitted no later than 21 days after employment and shall have been completed within 12 months prior to or 21 days after employing.
Evidence: Reviewed 4 staff records and found that Staff 2 and 4 had tuberculosis screenings completed over 12 months prior to employment.

Description: Based on review, the facility did not maintain children's records with the required information.
Evidence: Reviewed 5 children's records and found the following:
1) Child 1, 2,3,4 and 5 did not have documentation of their first day of attendance.
2) Child 1, 3 and 4 did not have documentation of work telephone numbers for one parent.

Description: Based on observation, the facility did not ensure that cleaning and sanitizing materials are not located above food, food equipment, utensils or single-service articles and should be stored in areas physically separate.
Evidence:
1) Observed in the Pre-K 1 classroom, hand sanitizer and Lysol spray stored next to single-service articles.
2) Observed in the Pre-K 2 classroom, dish soap and nail remove stored above single-service articles.

Evidence:
1) Observed in the Pre-K 1 classroom, an uncovered outlet on a surge protector near the teachers desk.
2) Observed in the Pre-K 2 classroom an uncovered electrical outlet on the wall by the counter with the sink.

Description: Based on observation, the facility did not ensure that tables are sanitized (22-VAC-40-185-10. Definitions: Sanitized means treated in such a way to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution. the surface of the item is sprayed or dipped into the disinfectant solution and allowed to air dry after use of the disinfectant solution) before and after each use or feeding.
Evidence: Observed that the staff were using Clorox wipes and not a solution that was sprayed or the table was not dipped.
