Comments:
An unannounced Monitoring Inspection was conducted today from TIME. There were 31 school-age to infants) directly supervised by 7 staff. The physical plant, 9 staff records, 5 children?s records, no children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, napping. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review, the facility did not ensure that children's records were maintained with the required information.
Evidence: Reviewed 5 children's records and found that none of them had documentation of the parent's annual review and update.

Plan of Correction: Will be completed the first week of January.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility did not ensure that areas and equipment of the center inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1) Observed on the school-Age playground, a large white tub with a broken corner and sharp edges.
2) Observed on the toddler playground a red tricycle with a broken edge near the front wheel.
3) Observed on the school-age playground slide a gap between the platform and the slide that can cause an entrapment or entanglement hazard.

Plan of Correction: The broken toys will be thrown away by tomorrow, and will submit a work order for the gaps in the slides.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility did not ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1) Observed in the preschool classroom a bottle of soap solution on a shelf and not in a locked place.
2) Observed adhesive spray accessible to children in an unlocked cabinet marked Keep Out of Reach of Children.
3) Observed in the 3-5 year old class, Lysol spray in an unlocked cabinet.

Plan of Correction: Corrected. Will memo staff tomorrow.
