Technical assistance today included physical environment.

Comments:
An unannounced monitoring inspection was conducted from 10:30 am - 2:30 pm. There were 49 children present with 7 staff. Many children are on vacation today. I observed children listening to a story, playing outdoors, talking about vacations and summertime, preparing for lunch and a calm naptime. Record keeping was well-organized. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation during today's inspection, there were some areas of the center not maintained in a safe condition.
Evidence:
Outdoors:
1. A padlock was observed at noon to be locked on the exit gate between the playground and the parking lot while children were in the building, preventing possible access/egress in the event of an emergency.
2. The bottom wooden boards of two metal fences were broken, leaving both fences with exposed protruding screws in the playground.
3. A metal fence was observed to be loose at the bottom, and the three inches or more space between the bottom of some of the fences and the concrete barrier posed a possible hazard. A wooden post and top of the wooden post were badly split in one corner.
Indoors: There were areas of chipping paint in the infant toddler room, and a cracked outlet cover in another room. i

Plan of Correction: 1. We have our health and safety coordinator unlock them each day, but while she is on vacation, the director or assistant director will make sure the padlock is opened.
2. and 3. A work order was put in on 5/17/19. I will put in another one and mark it urgent, and cover the screws with duct tape.
Indoors: A work order was put inf or that as well 6/20/19.

Standard #: 22VAC40-185-510-A
Description: Based on a review of medications and authorizations today, there were some forms that were not completed as required.
Evidence:
The prescription authorization for Child #2 did not give the school permission to administer the medication, and the second prescription authorization for Child #3 was not available today.

Plan of Correction: We will talk to the parents to get the correct form and find the prescription authorization or have the parent get another one.
