Technical Assistance:
We discussed the requirements for infant feeding schedules and outside maintenance. The risk assessments for violations were provided.

Also, we talked about requirements for parent communication of tummy time, injury records, emergency drills and diapering procedures.

Comments:
Thank you for your assistance during today's unannounced monitoring inspection conducted from 9:47 AM to 3:45 PM. Today, there were fifty-three children, ages infancy through school age, with eleven staff. I viewed program activities, staffing, supervision, interactions with children, equipment, agency inspections, insurance, posted information, infant care and records, emergency supplies and drill records, menu, nutrition, injury records, five records for children, five staff records, staff qualifications, staff training, indoor and outdoor areas. The children have enjoyed a variety of seasonal art activities. There were group story times, discussions and age appropriate learning activities. The infant caregivers responded to children's needs for feeding and comfort. Outside activities have been provided, weather permitting.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation of the outside playgrounds, and an interview with administrative staff, an area of the toddler playground was not maintained in a safe condition. Evidence: A metal strap located four inches above the concrete surface, that was used to wrap the gutter drain, was detached from the brick wall near the corner of the building. The strap area had a visible split where it had been secured to the building and sharp area at the opening for the hardware.

Plan of Correction: The assistant director will contact another administrative staff person about arranging for a maintenance worker to remove the metal strap. The metal strap will be covered with tape at one end and taped to the gutter drain. The teachers who supervised children on this playground will be told to keep the children away from the corner wall. Also, teachers will be reminded to report repair needs to administrative staff.

Standard #: 22VAC40-185-570-C
Description: Based on a review of records of children on formula, and interviews with staff, the center failed to obtain all required information for the records of infants. Evidence: The feeding schedule forms viewed for child 1F, 2F, 3F, and 5F did not include the brand of formula brought from home in bottles and fed by center staff and only said "formula". The feeding schedule form for child 4F only had information for feeding breast milk and the staff indicated that formula has been fed during recent weeks. The infant feeding schedule form for child 3F also lacked a schedule for feedings.

Plan of Correction: The infant staff will have parents write the brand and schedule on the forms that are missing information. The infant staff will check all feeding schedule forms to make sure they contain all required informaiton on the brand of formula and schedule for infants who are fed formula. In the future, the infant staff on duty will make sure parents update the forms whenever there are changes with formula use and schedules. The administrative staff will conduct periodic checks on the infant formula records.
