8. The diapering surface shall be used only for diapering or cleaning children, and it shall be cleaned with soap and at least room temperature water and sanitized after each use. Tables used for children's activities or meals shall not be used for changing diapers.

The licensing inspector conducted an unannounced monitoring inspection on Friday, January 3, 2020 between 9:00 am to 11:45 am to determine the center's compliance with licensing standards. During this inspection, the children in care were observed by the licensing inspector in the various classroom settings. The census for today's inspection consisted of 49 children with 10 staff present including administrative staff. The licensing inspector completed observations of the children in care completing a variety of activities to include engaging in free play at table with a variety of toys, asleep in cribs, sitting in age appropriate equipment, diapering by staff, participating in circle time, identifying colors, and eating snack at the table. The children's snack for today consisted of nutri grain bar, and apple juice. The licensing inspector completed observations of the school aged group at the center present for Winter break. The center's bathrooms, diapering stations, designated classrooms were inspected by the licensing inspector. The required posting were reviewed by the inspector. During the inspection there were eight children?s and five staff's records reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Standard #: 22VAC40-185-70-A Description: Based on review of staff records and interview with staff, the center did not ensure all staff records contained the following information.
Evidence: In Staff# 5's record (date of hire 8/30/19) there was documentation of one references found. Interview with staff confirmed only one reference for staff available for review during the inspection.Plan of Correction: The center will ensure prior to hire date there will be two references in staff records.
Standard #: 22VAC40-185-280-B Description: Based on observations, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence: In the Caterpillar 2 classroom, there was a closet unlocked containing two bottles of labeled cleaners on top of a shelf.Plan of Correction: The center will go over protocol with staff to ensure all hazardous materials are locked.
Standard #: 22VAC40-185-340-D Description: Based on observation, review of staff records, and interview with staff, the center did not ensure in each grouping of children a least one staff member who meets the qualifications of a program leader shall be regularly present.
Evidence:1. In Caterpillar 2 classroom, Staff#4 (date of hire 04/01/19) was observed with eight children in care.
2. Interview with staff confirmed position as an assistant and being in sole direct care of children during inspection observation and recently at the center with this group.
3. A review of Staff#4's record verified position as assistant and there was no documentation available for review showing this staff met lead staff qualification.
4. Interview with center's director confirmed this staff position and verified staff not meeting qualifications at this time.Plan of Correction: The center director will ensure there is a lead at all times with the assistant teachers.
Standard #: 22VAC40-185-500-A Description: Based on observation and interview with staff, the center did not ensure children's hands shall be washed with soap and running water after toileting and any contact with blood, feces or urine.
Evidence: In the Cuddlebugs classroom, diapering by Staff#6 was completed and there was no observation of child's hands being washed. Interview with staff confirmed that child's hand was not washed after diapering during the inspection.Plan of Correction: The center director will go over protocol for diaper changing with staff member today.
Standard #: 22VAC40-185-550-E Description: Based on review of emergency drills and interview with staff, the center did not ensure to maintain a record of dates of the practice drills for one year.
Evidence: During the inspection, there was no documentation found for a practiced fire drill in 12/2019. Interview with staff confirmed this drill was completed at the center but was not documented on the emergency drills log.Plan of Correction: The center director will ensure to log in emergency drill into the book once completed.
