Comments:
An unannounced monitoring inspection was conducted on 11/08/2019 in response to a self-reported incident regarding a child not arriving to the SACC program as scheduled. Based on record review, staff interviews, and a review of the center?s policies, violations are noted on the Violation Notice. Description: 22VAC40-185-(2)-40-E: Risk Assessment Rating: B2
Based on record review and staff interview, the center failed to ensure that the operational responsibilities of the licensee shall include, but not be limited to, ensuring that the center's activities and services are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards.
Evidence:
1. The ?Safety and Accountability? plan for employees provided by the SACC regional office states that ?Staff should contact the RS (Regional Supervisor) on Duty immediately anytime a child cannot be accounted for while at the center and while on field trips. This includes children who are scheduled to attend after school and are unaccounted for, children coming from other schools who do not arrive within a timely manner, children who do not report to SACC within a timely manner after a scheduled after school activity, and any children who are in attendance but cannot be accounted for at any given time and children who leave the direct supervision of SACC staff.
2. Staff #1 stated that ?Child #1 did not show up on November 1st, but Child #1?s name was on the roster for November 1st. I took the attendance that afternoon at around 4:20pm and Child #1 was not there so I checked with the office and they said Child #1 rode the bus home and had not been returned. They did not know Child #1 was supposed to be in SACC that day. They said Child #1 has been returned to school in the past if a family member was not at the bus stop. We continued on with our day.?
3. On Nov. 4, 2019, the RS was made aware that Child #1 was not accounted for on Nov. 1st when the parent of Child #1 contacted the RS to discuss the incident.
4. Staff #1 did not contact the RS on duty immediately to report that Child #1 was not accounted for.

Plan of Correction: Staff will follow SACC program policies and procedures at all times. Center specific procedures will be reviewed and revised as needed to ensure the accountability for children.
