Comments:
The licensing inspector conducted a subsidy health and safety inspection Friday, December 27, 2019 to review supplemental health and safety requirements. The inspector arrived at 9:45am and concluded the inspection at approximately 1:00pm. The center?s assistant director assisted the inspector throughout the inspection. The census for today?s inspection consisted of 18 children in the direct care of 5 staff members and one assistant director. Upon arrival, the children present were observed playing in different centers, interacting with staff, and peers. Each classroom was inspected and formal observations were later completed. The children were observed in their respective classrooms as they played with play dough, coloring, brushing their teeth, taking bathroom breaks, and during circle time. The center is equipped with age and stage appropriate materials and equipment for the children?s use. Staff were engaged with the children and were observed offering guidance when needed. All areas where the children receive care were inspected and found to be in compliance. The center had the following posted: license, daily schedules, emergency numbers, evacuation maps, menu, and various parent information. The center?s first aid kit and non-emergency supplies were inspected and found complete. Today?s lunch consisted of chicken nuggets, WG rolls, peas, carrots, apple sauce, and milk. During the inspection, five children?s records and five staff records were reviewed. Medication is administered, but there are no medications on site at this time. Transportation is provided and one van was inspected today. The most recent evacuation drill was documented on 12/24/2019. One shelter-in-place drill was documented on 03/10/19 and a second one on 12/24/2019. A lockdown drill has been scheduled by the end of December 2019.

A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on a review of five children's records and interview on 12/27/19, the vendor did not ensure to have a written allergy care plan for one child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: 1) The record for Child #4, enrolled on 3/6/17, indicated the child had a diagnosed food allergy, but there was no written allergy care plan. 2) During interview, a member of management reported the allergy care plan for Child #4 could not be located while the inspector was on site.

Plan of Correction: The assistant director will be in contact with the child's parent to see if they have a copy of the allergy care plan, if not, they will be requested to provide it by Tuesday.

Standard #: 22VAC40-665-540-A
Description: Based on a review of five staff records on 12/27/19, the vendor did not ensure one staff was evaluated by a health professional and was issued a statement that the individual was determined to be free of communicable tuberculosis (TB) within the required time frame.

Evidence: 1) The TB results in the record for Staff #5, hired on 6/5/19, were dated 6/11/19. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Plan of Correction: Moving forward, management will make sure TB results are submitted prior to employment and coming into contact with children.
