Comments:
An unannounced monitoring inspection was conducted from 8:55am-12:30pm. At the time of entrance, 75 children were in care with 26 staff members present. Children were observed during diaper changes, hand washing, reading books with the teacher, snack time, painting, singing, engaged in group activities at circle time and play time in the play gallery and multipurpose room. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, evacuation drills, attendance records, injury reports and the Emergency Preparedness Plan were reviewed. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review of seven children's records, the center did not obtain documentation of immunizations for a child prior to the child's first attendance.
Evidence:
1. Child #7 began at the center on 9/12/2019 and the center did not have the child's immunization record.

Plan of Correction: We received the immunization record during the inspection.

Standard #: 22VAC40-185-130-B
Description: Based on review of seven children's records, the center did not obtain documentation of updated immunizations once every six months for children under the age of two years old.
Evidence:
1. Child #4 (DOB 5/15/2018) was under the age of 2 years old and the immunization record the center had on file was dated 8/23/2018 and was not updated every 6 months.

Plan of Correction: The parent was contacted by email during the inspection.

Standard #: 22VAC40-185-140-A
Description: Based on review of seven children's records, the center did not have documentation of a physical in each child's file prior to or within 30 days of first attendance.
Evidence:
1. Child #7 began at the center on 9/12/2019 and the center did not have documentation of the child's physical.
2. Child #2 began at the center on 2/18/2019 and the center did not have documentation of the child's physical.

Plan of Correction: The parents were contacted by email during the inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center inside were not maintained in a safe and operable condition.
Evidence:
1. In room 105, the paint is chipping near the play kitchen. A lower cabinet labeled yarn and fabric had a large chip in the cabinet.
2. In room 121, the mirror is cracked on the wall. The crack is covered with tape.

Plan of Correction: The mirror was removed during the inspection and a request to repair, patch and paint was made.
