Comments:
Thank you for your assistance and your time during this unannounced monitoring inspection. The visit was conducted on December 12, 2019 from 10:42 am through 2:40 pm. There were thirty-two children in the care of seven staff. The total enrollment for the program was fifty-six children. There were six children records and five staff records reviewed. The medication and documentation were reviewed for three children. The exceptions to compliance were noted on the violation notice. Diapering procedures, hand washing, bathroom procedures, free play, circle time, reading, singing, outdoor walk, lunch, evacuation drills, daily schedule, transitions, and dismissal procedures were observed during this visit.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation and verification from staff and administrator, hazardous materials were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the Infant and Toddler classrooms, bleach solution was unlocked and hanging on the rack next to the diaper-changing table.
2. Hand sanitizer, rug cleaning product, and aerosol air freshener we unlocked in a cabinet in the toddler room. Each product stated: ?keep out of reach of children? and one or more of the following: danger; caution; warning; flammable; harmful if swallowed; causes burns; vapor harmful and poison.
3. Staff and administrator verified that the hand sanitizer, rug-cleaning product, aerosol air freshener, and bleach solution were not in a locked location.

Plan of Correction: Provider will ensure hazardous materials are locked.

Standard #: 22VAC40-185-510-C
Description: Based on a review of medication for children, medication policies and procedures, and an interview with the administrative staff, the centers policies and procedures for administering medication did not include a method to prevent use of outdated medication.

Evidence:
1. The center?s policies and procedures did not contain a method to prevent the use of outdated medication.
2. Administrative Staff confirmed that methods to prevent use of outdated medication was not included in the center?s policies and procedures.

Plan of Correction: Provider stated that she would have new methods written and put into her staff handbook by the time she returned back from vacation, if not before she leaves.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of staff records, and verification from administrator, the center failed to ensure that an out of state abuse and neglect search was requested within thirty days of employment.

Evidence:
1. An out of state abuse and neglect search was not requested for Staff #1. Date of hire for Staff #1 was 08/06/2019.
2. Administration stated that she was under the impression that the Fingerprint check now covered the out of state abuse and neglect search.
3. Administration verified that the out of state abuse and neglect search request had not been submitted within the required time period.

Plan of Correction: Provider stated that she was going to make contact on 12/13/2019 with the appropriate state to make a request.
