An unannounced monitoring inspection was conducted from approximately 7:00am-8:20am on January 9, 2020. Upon arrival to the facility, there were three staff present with three school age children in the cafeteria; throughout the course of the inspection there were 19 children present. Children were observed arriving to the program, playing with legos, board games and coloring sheets and talking freely amongst themselves. Five children's records were reviewed on this date. Licensing Inspector reviewed supervision, indoor areas, activities, equipment, nutrition, children's records, emergency evacuation and shelter-in-place drill documentation and procedures, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. Six medications were reviewed in the center on this date. Injury report documentation and procedures were reviewed. Exit interview conducted with the Recreation Supervisor.
Standard #: 22VAC40-185-60-A Description: Based on record review, in three of five children's records reviewed, the licensee did not ensure that each child's record contained all required information.
Evidence: The Recreation Supervisor confirmed that the following information had not been documented in each child's record:
Child #2 and Child #5-the names of any previous child day care and schools attended by the child
Child #4-work phone number for one parent
Plan of Correction: The Recreation Supervisor will contact the parents and have them to complete the missing information.
Standard #: 22VAC40-185-550-D Description: Based on review, the licensee did not ensure that a monthly practice evacuation drill was practiced at the facility.
Evidence: The Recreation Supervisor confirmed that there was no documentation of an evacuation drill practiced in December 2019.
Plan of Correction: The Recreation Supervisor will make sure that one is completed in January.
