Technical Assistance:
Reviewed the following with the Head of Operations:
- Documentation to keep when sending out the Search of the Central Registry for new employees
- Injury of a Child Requiring Outside Medical Treatment Online Report

Comments:
An announced renewal inspection was completed today. The physical plant, 6 staff files, 5 children's records, fire and health inspection, staff training, children's activities, medications, emergency policies, procedures and evacuation drills were inspected between 1:15-3:20pm. The outside play areas were not inspected today due to the weather conditions. There were 59 children under the supervision of 13 staff. Today, the children were observed enjoying free play, learning about icebergs, participating in circle time, listening to a teacher read a book and eating lunch. The children bring their lunches from home or can order from the food service program that the school utilizes each day they come to the center. The interactions between the children and staff were seen to be positive and constructive in nature. Should you have any questions, please contact me at mary.wizbicki@dss.virginia.gov.

Description: Based on file review, 1 out of 6 staff records does not have a TB test/screening completed within 2 years.
Evidence
1. Staff #5's most recent TB test/screening was completed on 8/23/17.

Plan of Correction: This will be completed again.

Standard #: 22VAC40-185-70-A
Description: Based on file review, 2 out of 6 staff records do not have all required information on file.
Evidence
1. Staff #3 and #5 do not have documentation that two or more references as to character and reputation as well as competency were checked before employment.
2. Staff #5 does not have documentation to verify that the individual possesses the education required by the job position.

Plan of Correction: These items will be obtained and kept on file.
