Technical Assistance:
Discussed sight and sound supervision in the the infant classroom to set up the room so all children are able to be seen during feeding and diapering. The classroom is a L - shape which causes blind spots .

Comments:
An unannounced monitoring inspection was conducted on January 9, 2020 from approximately 11:05 am to 1:44 pm. Six staff were present with thirty children. Additional staff were on site for kitchen and administrative duties. Five children's records and four staff records were reviewed. Children were observed eating lunch and preparing for nap time.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the licensee did not ensure hazardous substances be kept in locked place using a safe locking method that prevents access by children.

Evidence:
There was liquid soap in the toddler classroom labeled "Keep out of reach of children" and "caution." The liquid soap was not kept in locked place using a safe locking method . In the toddler bathroom, on a shelf unreachable for children there was a bleach and water spray bottle and insect spray labeled "Keep out of reach of children" and "caution." The bleach and water and insect spray was not kept in locked place using a safe locking method.

Plan of Correction: The vioaltion was corrected during the inspection. The soap was removed and put in the lock box. And the other items were kept in a locked box as well.

Standard #: 22VAC40-185-570-A
Description: Based on observation and staff interview, the licensee did not ensure when a child is placed in an infant seat or high chair, the protective belt was fastened securely.
Evidence: Three infants were observed eating lunch sitting at feeding/activity table without straps fastened.

Plan of Correction: Corrected during the inspection. Staff was spoken to about ensuring children are strapped at all times when sitting at the feeding table.

Standard #: 22VAC40-185-570-E
Description: Based on observation and staff interview, the licensee did not ensure the prepared infant formula shall be refrigerated, dated and labeled with the child's name.

Evidence: There was three bottles of infant formula that was not dated or labeled with the child's name. Staff #3 confirmed the bottles. were not labeled.

Plan of Correction: The violation was corrected during the inspection. Staff #3 cleaned the bottles and names and dates were washed off.
The center will start using tape and paper at the bottom of the bottle to label them.
