Technical Assistance:
The center is undergoing remodeling in some areas and new equipment is being installed in the playground. Make sure that all the wooden barriers in the playground that are being installed are free of splinters and cracked areas.

Comments:
An unannounced monitoring inspection was conducted from approximately 11:30 am-2:30 pm. At the time of entrance, 76 children were in care directly supervised by 15 staff members. Ratios were in compliance. Children and staff were observed during bottle-feeding, lunch, cleaning, transitioning to nap time and napping. The center looked clean and organized. Improvements to the physical plant were noted like new mulch, paint had been retouched, new cabinets in some rooms and new latches for cabinets and closets. A selection of staff records, the physical space and required posted documents were reviewed at this time. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-240-A
Description: Based on record review and interview, it was determined that the staff did not receive training in the center's playground safety procedures.
Evidence:
1) The records for Staff 1 - 8 did not contain documentation of having completed orientation training in the center's playground safety procedures.
2) The center's director stated that all employees complete a training on general supervision and safety procedures but a center-specific training has not been put in place.

Plan of Correction: I will create a new bulleted list that includes playground safety training for our orientation training.

Standard #: 63.2(17)-1721.1-B-3
Description: Based on record review, the center failed to obtain Central Registry results from VA and out of state.
Evidence:
1) The record for Staff #1 (start date 8/12/2019) Sworn Statement indicated residency in the state of
Kentucky within the previous 5 years. Staff #1 did not have documentation of a Central Registry
Background check result in the state of Kentucky.

Plan of Correction: I will send an email today.
