Comments:
The following inspection took place between the hours of 9 am through 12 pm (noon). Today, I met with the Staff of Villa Montessori Preschool located in the town of Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 34 children observed with 8 classroom staff; good ratios in all 4 rooms. A total of 5 children?s files and 5 staff files were reviewed. The children were engaged in the following: small motor skilled activities, center activities within the classroom, snack, bathroom breaks, hand washing, circle time, large motor skilled activities outside on center playground, and other organized activities. If you should have any future questions, please call or send an e-mail to charles.perkins@dss.virginia.gov. Thank you.

Violations:
Standard #: 63.2(17)-1721.1-B-3
Description: Based on review and interview, 1 of 5 staff files indicated that the child day center did not obtain a copy of the results of a search of the central registry (maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry) maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him or her.

Evidence -
1. On the date of inspection (01/08/2020), on a Sworn Disclosure Statement (SDS) dated - 07/05/2019, to the question: Have you lived outside of Virginia in the past five years? Staff #1 checked "Yes" and listed both Florida and Michigan.
2. Documentation from Florida was provided. However, an out-of-state CPS check from the State of Michigan was not available for review.
3. Staff #1 stated that she resided in the State of Michigan from January through July of 2015.
4. This same standard was previously cited during an inspection conducted on 07/23/2019.

Plan of Correction: A request for this information shall be sent to the State of Michigan within the next 10 days. Verification that this request was sent shall be given to our Licensing Inspector (LI). Once this information from Michigan has been obtained, the LI shall be contacted and the documentation shall be reviewed.
