Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and a review of the annual process of updating the contact and emergency information on file for each child.

Comments:
The following inspection took place between the hours of 9 am through 1:30 pm. Today, I met with the Staff of the Montessori Academy at Belmont Greene located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 123 children observed with 18 staff; good ratios in all 7 rooms. A total of 10 children?s files and 7 staff files were reviewed. The children were engaged in the following: circle time, large motor skilled activities outside on center playground, diapering, hand washing, bathroom breaks, center activities within the classroom, story time, learning, lunch, large motor skilled activities in the gym, nap time, and other organized activities. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review, 1 of 10 children files indicated that each child did not have a physical examination (by or under the direction of a physician) within one month after attendance.

Evidence -
1. On the date of inspection (01/09/2020), documentation of a physical examination for Child #4 was not available for review.
2. The start date for Child #4 was listed as 09/06/2014.

Plan of Correction: This information shall be obtained and placed on file for Child #4.

Standard #: 22VAC40-185-60-A
Description: Based on review and interview, 1 of 10 children files indicated that the center did not maintain a separate record for each child enrolled.

Evidence -
1. On the date of inspection (01/09/2020), a file for Child #1 was not available for review.
2. Administrative Staff stated (during an interview conducted on the date of inspection) that the information and documentation for Child #1 was still in the process of being collected. Some information needed in case of an emergency has already been collected.

Plan of Correction: All applicable information and documentation needed for Child #1 shall be obtained and placed on file.

Standard #: 22VAC40-185-270-A
Description: Based on observation, one area inside the center was not maintained in a safe condition.

Evidence -
1. On the date of inspection (01/09/2020), inside the gym, the front lower wall (off of the hallway with the odd numbered rooms), two holes were observed.
2. Both holes were located near the trim that runs along the wall next to the floor. The first large hole (located behind the door) was approximately 11 inches in width, and 7 inches in height. The second smaller hole (located directly above the trim) was approximately 2 inches in width and 1 inch in height.

Plan of Correction: This area shall be addressed and both holes shall be repaired to prevent access.
