Technical Assistance:
During inspection documentation of medication authorizations were discussed. management was informed to have each medication on its own authorization form.

Comments:
An unannounced renewal inspection was conducted today from approximately 12:05pm-2:35pm. There were 84 children (infants-5yrs.) directly supervised by 13 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, eating and resting. Diapering and hand-washing procedures were also observed. There was an adequate supply of materials for the children. The required number of staff with current certification in MAT, CPR and First Aid, as well as Daily Health Observation training were met.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of Children's records it was determined that all required information was not maintained.

Evidence: During inspection Child A and Child B did not have the work place and number for 1 of 2 parents listed.

Plan of Correction: There has been a change in our enrollment forms we will get the proper information.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined that all parts of the facility were not maintained in safe and operable condition.

Evidence: During inspection the ceiling the toddler room appeared to have a black water stain and was chipping.

Plan of Correction: There has been a request made for the necessary repairs.

Standard #: 22VAC40-185-350-E-4
Description: Based on observation it was determined that a 1 : 10 ratio was not maintained in the transitional kindergarten classroom.

Evidence: During inspection there were 17 children in the classroom with 1 teacher; Children were eating, using the bathroom and resting on their mat.

Plan of Correction: Corrected during inspection as 2nd teacher returned from emptying the trash.
