Technical Assistance:
Discussed the injury prevention plan and accident report requirements.

Comments:
An unannounced renewal inspection was conducted today beginning at 2:00 pm. At the time of entrance, 49 children were in care with 3 staff members present. The physical space, emergency supplies, evacuation drills, attendance records, injury reports, medications, the Emergency Preparedness Plan, 5 children's records and Staff certification for CPR/First Aid and Mat training were reviewed. Ratios were in compliance and required documents were posted. The site was clean and organized. The children were observed playing games, reading, coloring and playing on the computer. Interactions between the children and staff were positive. Areas of non-compliance are identified in the violation notice. Description: Based on review of the written records of children's serious and minor injuries, the center did not have documentation of Staff and parent signatures or two staff signatures on each report.
Evidence:
1. Five children's accident reports (dated 9/11/2019, 9/13/2019, 9/25/2019, 9/25/2019 and 9/30/2019) did not have staff and parent signatures or two staff signatures. The reports had one staff signature only.

Plan of Correction: Going forward, I will make sure we have two staff signatures or the parent signature with a staff signature.
