An unannounced inspection was conducted in response to a self-reported incident that occurred at the center on 12/19/19 regarding allegations in the areas supervision and physical plant. Interviews were conducted with staff, records reviewed and video footage of the incident was viewed. A violation has been cited pertaining to the allegations.

Standard #: 22VAC40-185-270-A
Description: Based on a records review, interview and observations made of the program, the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence: On 12/19/19, three year-old child #1 sustained scrapes and broken skin on her left ring and pinky fingers, after an unsecured cubby shelf fell over and landed on the mentioned two fingers. While playing hide-n-seek child #1 hid behind the cubby. Loosing balance, child #1 fell back and the cubby fell over onto her fingers.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
The director has since removed the cubby fro the center and is working on securing all remaining cubbies.
