Comments:
An unannounced Monitoring Inspection was conducted on January 9, 2020 from approximately 10:20 am until 1:50 pm. Upon entrance there were 85 children and 14 staff present with children. Additional staff were on site for administrative purposes. The record review sample consisted of 9 children's records and 6 staff records. Children were observed writing in journals, watching a video, preparing for and having lunch. Two medications were reviewed. Areas of noncompliance are noted on the violation notice. The results of the inspection were reviewed and verified with the center director during the exit interview.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the licensee did not ensure the areas of equipment inside and outside of the center were maintained in a safe and operable condition.

Evidence: On the toddler playground, inside the plastic play house there are two areas where the plastic is cracked and broken. The areas measure between 1 and 3 inches long.

Plan of Correction: The center director/owner will repair the playhouse.

Standard #: 22VAC40-185-280-B
Description: Based on observation and staff interviews, the licensee did not ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the Bricks 1 room there were two containers of disinfectant spay (green liquid) sitting on the cubby shelf. In the Windows II room in a bottom unlocked cabinet there was a container of hair conditioner. Both items are labeled "keep out of reach of children" and at least one other statement "caution", "warning" "hazardous" and "flammable".

Plan of Correction: The center director/owner will fix the lock on the cabinet and will remove the hair conditioner today.
