Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:15 am - 12:30 pm. There were twenty five children in care: five infants with two staff, nine toddlers with two staff and eleven preschoolers with two staff. The children were observed playing with play dough, moving to music, at story time, during lunch and participating in a fire drill. Five staff's records and five children's records were reviewed. Violations from the previous inspection were also reviewed and one was repeated. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-60-A
Description: Based on record reviews, the center failed to obtain all required information for the children's records.
Evidence:
1. The inspector reviewed five children's records. One of five records reviewed (child record #4) did not contain street addresses for the emergency contacts. One of five records reviewed (child record #3) did not contain proof of identity and age and child's date of enrollment was 04/11/2019.

Plan of Correction: The director will request this information from the children's parents and monitor the children's records to ensure all information is complete.

Standard #: 22VAC40-185-70-A
Description: Based on record reviews, the center failed to obtain all required information for the staff's records.
Evidence:
1. The inspector reviewed five staff's records. One of five records reviewed (staff record #4) did not contain documentation that two or more references were checked before employment began. Staff #4's first date of employment was 10/08/2019.

Plan of Correction: The director of human resources will obtain the references and maintain in the staff's record.
