Technical Assistance:
1. Reviewed written attendance record requirements. 2. Discussed/triained vendor in new subsidy requirements. 3. Model forms provided: emergency plan, and proof of identity.

Comments:
10:10-11:25am Conducted an unannounced annual subsidy inspection. Observed 2 children in care + 2 children that live in the home = 5 points with the vendor. Ratios and supervision were in compliance. The children were playing with toys, watching child appropriate shows on TV, and eating a snack of fruit and yogurt. The vendor participates in the USDA Food Program. The home was found to be clean and sufficiently supplied with toys and equipment for the children. The vendor is currently certified in CPR/First Aid. The vendor speaks minimal English, and her daughter translated communications between vendor and inspector. One administrative and one safety non-compliance found.

Description: Based on records reviewed, it was determined that a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: (1) The attendance records did not reflect when Child #2 and #4 left for and returned from school each day. (2) There was no written attendance for this week.

Plan of Correction: The vendor said she will write the attendance times for each day.

Standard #: 22VAC40-665-240-C-2
Description: Based on observations made, it was determined that electrical outlets did not have protective covers. Evidence: One outlet in the bathroom, and another in the living/dining area did not have covers today. Both of these areas are accessed by children.

Plan of Correction: The vendor put covers on the outlets during the inspection.
