An unannounced monitoring inspection was completed on 01/13/2020 from 9:40 A.M.-2:00 P.M. At the time of the inspection 52 children were in care with nine staff and one administrator present. Six children records, six staff records, one medication and required documentation, five injury reports, evacuation drills, first aid kits, emergency supplies, playground area, health inspection, fire inspection and posted required information were reviewed. Children were observed during transition to outdoor play time, outdoor play time, morning work cycles that included number recognition, geography, and literacy activities. The children were also observed during fine motor activities, music, movement, handwashing, lunchtime, diapering, and naptime. The exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-185-140-A Description: Based on review of children files, the center failed to obtain a physical examination within 30 days of attendance.
Evidence:
1. Child #4's physical examination was dated 06/11/2018. Date of enrollment for Child #4 was 03/01/2018.
2. Staff verified the date of enrollment for Child #4, and the date the physical examination was obtained.Plan of Correction: The parents stated they had submitted the physical exam. Administration could not locate the exam and requested another one be completed.
